For many of us at Foundation Medicine, cancer has directly touched our lives, either through our friends, our family, and in some cases, ourselves. So much of the passion that goes into our work each day comes from our personal experiences as patients, caretakers and supporters, connecting us to the company’s continued commitment to transform cancer care. Many of you have seen the photos featured on our Patient Walls in Cambridge, RTP and Penzberg, as well as on our careers website. These photos feature members of the Foundation Medicine community – from employees to loved ones, as well as patient advocates and caretakers – that serve to highlight these personal connections we have to cancer. We are proud to announce that this fall we will be installing a Patient Wall on the 3rd Floor in our Boston headquarters. The installation of this Patient Wall is meant to provide us all with a daily reminder of why we work at Foundation Medicine, with stories of hope and inspiration, fueling our passion and our pursuit to bringing personalized treatment options to more people living with cancer.
We will be hosting a photoshoot on the 16th floor of HQ on July 16th and 17th. We have chosen this date because it coincides with the Clinical and Biopharma Mid-Year Meeting being held in Boston which will allow us to include photos of remote field sales employees.

The following two forms will need to be completed before we can display your photo:
PLEASE DOWNLOAD A COPY INSTEAD OF EDITING THE FORMS WITHIN THE LINK :)
1. Model Release and Consent - employee to sign at HQ before they get their photo taken. Remote employees, email to passion@foundationmedicine.com
2. HIPAA Authorization form - loved one to sign (or employee if they have been impacted by cancer and not sharing a photo of a loved one). Email to passion@foundationmedicine.com
Note: a personal representative can sign this HIPAA authorization on behalf of their loved one;
what a “personal representative” actually means under HIPAA (e.g., healthcare power of attorney, executor or administrator of estate, or some other legal instrument that allows the signatory to make healthcare decisions on behalf of the loved one); and
with respect to a deceased loved one, the person signing the HIPAA authorization must be a personal representative.
