Sign-On Bonus Documentation Requirements

As a candidate for Foundation Medicine, please review the following important information regarding our sign-on bonus documentation process. We kindly ask that you do not submit any sign-on bonus documentation to your recruiter until it is specifically requested.

Documentation will be required only at the end of the interview process.

Acceptable documents for financial incentives, including bonuses:

- Offer letter
- Plan document
- Equity statement from a third-party provider to show grant dates, vesting dates, price, number of shares, etc. (if applicable)

Details that should be included in the documents:

- Your Name
- Company Name/Branding
- Bonus Amount (% bonus target and base salary)
- Payment Schedule (payment dates or bonus year start/end dates)
- Forfeiture Clause: Confirmation that bonuses are forfeited if employment ends before the payment date.

If the documents contain personal information such as your social security number, you may black that out.
